
New Patient Registration

Register online or come in to the surgery with your relevant Identification
We accept new patients who live in our practice area. You can register online or by completing a registration form which can be obtained from the practice or downloaded from our website. When you register, we will offer you an appointment with our HCA for a new patient health check. This helps us make sure that our records are accurate and that all routine checks to promote your health and wellbeing (such as blood pressure/cholesterol/smear tests/mammograms) are up-to-date.
If you want to join the practice please complete a registration form and bring the completed form along to reception with an address I.D. and a photographic I.D. (mobile 'phone bills are not accepted). Once your registration is completed, your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

- Patient access (on-line booking please ask at reception for your unique code to access this service).
You may request a pre-bookable appointment with a doctor for a future date.
Star Lane Medical Centre assures our patients that, if they have a medical emergency. they will be advised how to access a clinician on the same day. Patients may request to see a doctor of their choice, which the Practice will endeavour to accommodate, when possible.

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.

Need Telephone Advice

If you require advice from the doctor or nurse please ring as early as possible on a weekday. You will be asked to leave a contact number and the doctor will return your call by the end of the day.
If you 'phone after 12:00, you will be put on the duty doctor's list for a call back.
